4.个人简历(中英文) Curriculum Vitae

5.学历证明附成绩单(原件+ 汉语翻译公证件)Degree diploma certificates、transcript and Learning evaluation（the original copy and Notarized copy in Chinese）

6.健康体检证明Foreigner physical examination form

7.未满18岁学生提供在华监护证明Letter of Guarantee(under the age of 18)

8.证件照（ID photo）：

要求：⑴以护照号码命名（name the photo after your passport number）

⑵照片参照格式：彩色2寸免冠证件照，白色背景无边框，头部占照片尺寸的2/3，照片尺寸不低于320*240像素，高宽比为4：3， 大小为100-500KB，JPG格式（the format of the photo should be :2-inch bareheaded id photo with a white background without border, the head makes up 2/3 of the photo, no less than 320*240 pixels in size, 4:3 in aspect ratio, 100-500KB in JPG format）

9.三分钟内自我介绍的视频，内容包括个人形象展示、基本信息、兴趣爱好、学习计划等，中英文（A three minute self introduction video, including personal image display, basic information, interests and hobbies, learning plans, etc., in Chinese or English）

10.HSK证书（HSK certificate）

11.学习计划（Study Plan）

12.两名专业领域推荐人（需副教授或教授）：Two professional field recommenders (associate professor or professor required)

注：10-11为申请本科生需提供，10-12为申请研究生需提供。

NOTE：10-11 Provided by undergraduate，

      10-12 Provided by postgraduate.
